Mandatory Pediatric ACGME Procedure Logs

There are two ways to access the online ACGME Procedure Log.

1) One way is off the hospital clinical workstation. Just go to the homepage on any hospital computer. Four lines above the "Text Paging" option is "Pediatrics Procedure Recorder".

2) Another option is to use the following link: https://www.acgme.org/residentdatacollection/

For either option:
user ID: first initial and last initial followed by the last four digits of your social security number
password: first initial and last name. (note: for interns, your password is yalepediatrics)

The system is case sensitive.
